You can create a self-assessment survey in a specific content area of your course. You can create the self-assessment survey at the top level of an existing content area or you can drill down into a chapter folder in an existing content area.
To create a self-assessment survey in a content area:
Click the course name on the My CourseCompass page.
Click Control Panel.
Under Content Areas, click the link for the content area where you want to create the self-assessment survey, for example, Course Documents or Assignments. You can create your survey at this level, or you can navigate to a chapter folder or add a new folder.
Select Self-Assessment Survey from the Select drop-down list on the far right, then click Go.
Click the Create button. The self-assessment survey Info page appears.
Enter a name, a description, and instructions for your students.
Select a format. No Sections is selected by default.
If you want to create a self-assessment survey having multiple sections, refer to Creating a self-assessment survey: Multi-Section format.
Click Submit.
Enter feedback in the Feedback Text textbox.
Select the feedback type that students will see upon completion of the survey.
Select Tallied Score if you want the students' total scores to be computed and displayed to students.
Select Submitted Answers/Question Feedback if you want students to see their responses together with feedback on individual questions.
Click Submit. The Self-Assessment Survey Canvas appears.
Add questions. Click Submit after creating each question. The Self-Assessment Survey Canvas refreshes.
When done adding questions, click OK.
Select the newly-created self-assessment survey from the Add self-assessment survey list and click Submit. The self-assessment survey Added page appears, confirming that the survey has been added to the content area.
Click OK.
After clicking OK, the Modify self-assessment survey page appears. From here you can modify the self-assessment survey or modify the self-assessment survey options.
